- scan_uploads.php now falls back to UPLOAD_DIR/META_DIR from config.php

- Resolves paths from CLI flags, env vars, or config constants (UPLOAD_DIR/META_DIR)
- Idempotent; safe to run at startup via SCAN_ON_START
